A Superbill will be provided upon request for you to submit to your insurance company for reimbursement. Blended Pathways therapy does not assist with this process, as insurance companies have individual requirements. Please contact your insurance company with any questions.

No insurance, no problem! A credit card is required to be on file and will be charged on the day of your scheduled appointment.
